Eligible voters can request a mail ballot without using a formal application; instead, they can send an informal application by mail or in person. The request should include:
- Your name and the address where you are registered to vote;
- Your driver’s license or personal identification card number issued by DPS;
- If you have not been issued one of those numbers, you can provide the last four digits of your social security number (SSN).
- Or you can write a statement that you have not been issued either of those numbers.
- The address where the ballot should be mailed.
- The reason you are eligible to vote early by mail;
- The election date and the specific election you are requesting a ballot for (for a primary election, specify the political party’s primary – Republican or Democratic);
- Your signature (or a witness’ signature if you cannot sign).
